Scholar Self-Certification and Verification A completed one-time COVID-19 Self-Certification and Verification Form is required for every scholar to begin school. It must be completed prior to the first day of school and only needs to be completed one-time.
This is a form that describes the daily COVID-19 screening you will conduct each day at home, confirming that your child does not have any COVID-19 symptoms or other exposures before you send your child to school on any given day.
Please find the symptom screening process outlined below. For parents/caregivers who wish to complete a paper form, these are available at your scholar’s school. Parents may request copies of paper screeners from their school. However, a form must be completed prior to a scholar attending school on the first day.
If a scholar arrives at the school on the first day with neither the one-time online or paper form complete, staff will call a caregiver and complete a paper form over the phone. In an instance when a caregiver is unavailable, a staff member will complete the form and take the scholar’s temperature. If the scholar is cleared and has a temperature under 100.4, they may enter the school building. If they do not, they must return home. The scholar will be sent to the school’s designated Care Room while a staff member contacts a caregiver to pick up the child. COVID-19 Screening for Visitors
Whenever possible, parents, guardians or other visitors should schedule meetings or appointments in advance. We greatly appreciate your partnership in supporting this scholar and colleague centered safety precaution. All visitors must sign-in to the facility to verify that they have read and will abide by the entry requirements posted at the entrance.
Please note that other risk mitigation practices detailed on our Hub, including required mask wearing, social distancing whenever possible, cleaning, disinfecting, maintaining air quality and available hand sanitizing stations remains standard protocol at all Acero Schools facilities.
